Personal Training Forms

In order to create a workout plan that is best for you, I need to know more about you.

These forms will give me an understanding of your health and exercise history, your goals, your likes and dislikes in terms of different modes of exercise, and what exercise equipment you have available to you to use with your plan. I work with my clients to achieve their goals, not mine, in a safe, effective, fun manner.
